Reports to the Director of Care
the RAI Coordinator is accountable for the Resident Assessment Instrument – Minimum Data Set 2.0 (RAI-MDS) program to support the delivery of optimal care and service, based on legislative requirements and organization standards. This position acts as the RAI-MDS resource for all participating team members in the home; Manages the coordination and completion of all components of the program; Establishes, maintains and communicates the current RAI-MDS schedule to the interprofessional team; Produces monthly reports.
Credentials: Current general certificate of registration with a college of a regulated health profession or Ontario College of Social Workers and clinical, project experience. Current AIS Certification. Successful annual completion of RAI MDS certification per Ministry of Health requirements for AIS • RAI-MDS education is an asset. Current license to practice. Familiarity with RAI-MDS 2.0 preferred.
