Community Paramedic
Reporting to the Commander of Community Paramedicine, the Community Paramedic provides community-based health care services to the County’s residents, often working with vulnerable, frail or marginalized populations to bridge gaps in local healthcare service delivery. Responsibilities include examine, assess and screen individuals to coordinate and initiate healthcare services with existing medical and other health care service providers. Complete overall health appraisals and applicable assessments, including but not limited to falls, home safety, medical, and medication reviews. Conduct regular home visits and closely monitor patient health care needs and disease progression. Perform urgent home visits as needed and in keeping with patient’s goals of care to remain safely at home. Act as a healthcare educator to patients and their caregivers including such topics as chronic disease management, falls prevention, palliative care, etc. Monitor and follow up on client activities (e.g., after hospital discharge, health status changes).
